The Perfect Box means the Lego set has not been opened which just like the brand new packaging in the store and does not has any damage. Unopened means that the seals on the Lego packaging are all intact and have not yet been opened.

Usually the price will be higher if the Lego set is still in perfect box or unopened conditions.
